L2T Overview

L2T goals are to engage and link individuals newly diagnosed with HIV/HCV to treatment and/or support individuals to re-access treatment. Our program provides client-centred case management and evidence based interventions to promote and monitor treatment adherence.  We recognize that treatment regimens are complex and impact health and wellness therefore, L2T welcomes community partners’ involvement for a holistic, connected, care team approach.

As part of the Province’s STOP HIV/AIDS initiative, L2T provides care, connection and coordination for eligible and referred individuals with HIV/HCV through a coordinated, integrated multidisciplinary case management service.  This initiative is part of a province wide effort to reduce transmission of HIV and HCV and engage HIV/HCV positive individuals into treatment.

Program Eligibility

  • Newly diagnosed with HIV and/or HCV
  • Re-engagement into care for HIV and/or HCV positive individuals
  • HIV and/or HCV positive individuals who would benefit from strengthening their engagement into care
  • Able to provide legal consent >16yrs

Duration of Program

  • 3-9 months
  • Intensive case management support
  • Providing Care, Connection and Coordination for HIV/HCV treatment.
